BEFORE, DURING AND AFTER

What you can manage in MyMediBay.

01 / PREPARE

Prepare for your appointment.

Appointment details, preparation tasks, relevant history, medicines, allergies and support needs help the care team prepare. A Vitals Camera reading can be requested where appropriate to the service.

02 / UNDERSTAND

Understand your reviewed findings.

Bring the visit summary, reviewed results and agreed care plan together. Know whether an action is yours, whether the team is reviewing something, or whether another service needs to respond.

03 / CONTINUE

Follow your care plan.

Track appointments, clinician-directed medication information, referrals and reminders. A referral sent, a receiving-service update and a confirmed appointment are different stages—not the same notification.

CARE CIRCLE

Help someone you care for.
Keep their record their own.

The planned Care Circle allows an authorised parent or carer to move between linked profiles. The selected person remains clear, with their own appointments, information and care plan.

Access must be based on the appropriate consent and legal authority. Caring for someone does not automatically give unrestricted access to their record, and permissions may change as circumstances change.

One patient. One connected record.Linked support should never mean mixing one person’s results, medicines or appointments with someone else’s.
